Wildlife mortality monitoring Form to gain insight in dead wildlife in the Netherlands. Monitoring death in wildlife Use this form to report the death of a wild animal or bird. This will be used to gain insight into causes and rates of mortality in wildlife. You are asked to provide only minimal personal data, and this will only be used to contact you in rare cases when more information is required (for example, the death of a rare species).
